I’m David Spielman, and I personally handle all adult autism and ADHD assessments at Quirkiatry. I’m not just a psychiatric nurse practitioner—I’m also autistic and have ADHD myself. That gives me a very different lens than most clinicians when it comes to understanding what neurodivergence actually looks like in real life.
I know how easy it is to “pass” on paper while still struggling internally. I know how masking, compensation, and chronic self-monitoring can confuse standardized tools. I know what it feels like to not quite fit any mold—and how invalidating it is when a provider overlooks your reality because you’ve adapted too well.
So when I’m assessing you, I’m not just checking symptoms—I’m reading between the lines. I know the red flags that don’t show up in test scores. I understand the contradictions and complexities because I live them too. That means I can offer something rare: a nuanced, respectful, and accurate evaluation that actually reflects who you are—not just how you perform in a clinical setting.

How Psychiatric Assessment Is Different
As a psychiatric nurse practitioner, I approach assessment through both a clinical and functional lens. I’m not just asking, “Do you meet criteria for ADHD or autism?” I’m asking, “What’s making life hard right now? What’s getting missed? And what’s actually going to help?”
That includes looking at:
Physical health factors like sleep, chronic pain, diet, or medication effects
Medical comorbidities that can mimic or complicate neurodivergence (like anxiety, OCD, trauma, or autoimmune conditions)
Cognitive and emotional patterns through psychological testing
Real-world impact—what’s showing up in work, relationships, and executive functioning
The goal isn’t just a label. It’s clarity. It’s context. It’s a treatment roadmap rooted in both medical science and lived understanding. Whether that means confirming a diagnosis, identifying overlapping conditions, or finding new paths forward, this process is about seeing the full picture—so we can actually do something with it.
What Happens After Testing
Once your assessment is complete, we don’t just hand you a report and send you on your way. We schedule a dedicated consultation to walk through your results together.
During this session, we’ll review your report in plain language—what each result means, how the pieces fit together, and what it all says about how your brain actually works. We’ll talk about any diagnoses, but we’ll also focus on the real-life implications: what’s getting in your way, what’s working for you, and what supports might make things easier.
If you’d like to keep working together, we can build a personalized treatment plan tailored to your goals. That might include medication, coaching, therapy referrals, executive function strategies, or other forms of support—all aligned with how you think, process, and live.
The end goal isn’t just understanding yourself better. It’s using that understanding to create meaningful, practical change.

We start with a deep dive into your medical and psychological history, then guide you through a curated set of tools that evaluate attention, executive function, emotional regulation, and related traits. These include standardized measures like the ASRS, DIVA-5, CAARS-2, and performance-based tasks through Creyos. Because ADHD often coexists with autism and OCD, we also include screeners like the RAADS-R, AQ, and Y-BOCS to give us a full picture. Afterward, we synthesize your results into a clear, practical report with diagnostic impressions and actionable next steps—whether that’s accommodations, coaching, or treatment. Our goal is to give you the insight and clarity to move forward with confidence and self-understanding.

We also screen for co-occurring conditions such as ADHD, OCD, and mood disorders using validated measures like the ASRS, Y-BOCS, and PAI. Once complete, your results are compiled into a detailed, user-friendly report with diagnostic impressions and practical next steps.
